Autonomous APRN Practice — Patient Disclosure

Required notice pursuant to Florida Statute § 464.0123

Teresa Vazquez, FNP-BC, APRN is an Advanced Practice Registered Nurse (APRN) licensed by the Florida Board of Nursing and practicing under autonomous practice authority as granted by Florida Statute § 464.0123.

As an autonomous APRN in Florida, Teresa Vazquez provides primary care services — including family medicine, ongoing health management, acute illness, preventive care, and health education — without physician supervision or collaboration, within her certified scope of practice.

Scope of this practice includes: Family and primary care medicine, ongoing condition management (hypertension, diabetes, cholesterol, asthma/COPD, etc.), acute and sick visits, women's health, men's health, weight management, medication management, and preventive care for adults and children.

This practice does not provide: Psychiatric evaluation or treatment, controlled substance prescribing for mental health conditions, or any service outside the autonomous primary care APRN scope defined by Florida law. Patients requiring psychiatric or specialist care will be referred to the appropriate provider.
