Complete our short patient screening form at terryhealthclinic.com/get-started. This takes about 2 minutes. We collect basic information to confirm you meet our eligibility requirements — Florida resident, no active insurance, age 7 or older.
Within 24 business hours, Teresa or a staff member reviews your application. If you qualify, you will receive a patient portal invitation email with instructions to set up your secure account. The TerryHealth patient portal is invite-only — we do not accept self-registration.
Once your portal account is active, you can book your free New Patient Consultation. This 30-minute video visit is completely free — no credit card required for this first visit.
If you decide to become a TerryHealth patient after your free consultation, the next step is your New Patient Evaluation — a one-time $100 clinical visit. This is your first full clinical assessment, paid once when you commit to care, before any ongoing visits begin.
All TerryHealth visits are conducted through CharmHealth's secure, HIPAA-compliant video platform — no app download required. Your appointment link appears directly in your patient portal.
After your New Patient Evaluation, you'll decide how you want to receive ongoing care. You can pay per visit or enroll in one of our monthly membership plans for better value and more visits included.
As an active TerryHealth patient, you have consistent access to Teresa through scheduled telehealth visits and secure portal messaging. Prescriptions are sent electronically to your preferred pharmacy. Lab orders go to Quest or LabCorp.

Florida driver's license, state ID, or passport. Required for identity verification before your first clinical visit.
A list of all current medications, doses, and frequencies. This helps Teresa provide safe, coordinated care from day one.
Credit or debit card saved on file in your patient portal. Used for automatic prepayment 4 hours before visits.
Complete all required intake forms in your patient portal before your first clinical visit. Takes about 10 minutes.
