TerryHealth LLC uses electronic communications and CharmHealth EHR, a HIPAA-compliant electronic health records platform, to manage your health information. All telehealth sessions are conducted through Charm Integrated Video, a secure browser-based platform. TerryHealth LLC is a direct-pay telehealth practice serving patients without active health insurance located in the state of Florida.
TerryHealth LLC maintains a record of all healthcare services provided to you. You may request to see and copy that record through your patient portal at terryhealthclinic.com.
Your protected health information may be used and disclosed by Teresa Vazquez, FNP-BC, TerryHealth LLC staff, and others outside of our practice who are involved in your care for the purpose of providing healthcare services, supporting our practice operations, and for other uses required by law.
We will use and disclose your protected health information to provide, coordinate, or manage your healthcare and any related services. For example, your health information may be shared with a specialist or laboratory to whom you have been referred.
TerryHealth LLC is a direct-pay practice that does not bill insurance. Payment is collected directly from patients. Your health information may be used to process your direct payments and generate receipts or superbills upon request.
We may use or disclose your protected health information to support quality assessment, licensing, and other business activities necessary to operate our practice.
We may use or disclose your protected health information in the following situations without your authorization: as Required By Law, Public Health Issues, Communicable Diseases, Health Oversight, Abuse or Neglect, Food and Drug Administration requirements, Legal Proceedings, Law Enforcement, Coroners, Funeral Directors, Organ Donation, Research, Criminal Activity, Military Activity and National Security, Workers' Compensation, and Inmates.
You have the right to inspect and copy your protected health information. Much of your health information is available directly through your patient portal at terryhealthclinic.com. For records not available through the portal, please contact us at appointment@terryhealthclinic.com.
You have the right to request that we restrict the use or disclosure of any part of your protected health information. TerryHealth LLC will consider all requests seriously but is not required to agree to all restrictions.
You have the right to request to receive communications from us by alternative means or at an alternative location.
You have the right to receive an accounting of certain disclosures we have made of your protected health information.
You may have the right to request that TerryHealth LLC amend your protected health information. If we deny your request, you have the right to file a statement of disagreement.
You have the right to obtain a paper copy of this notice upon request even if you have agreed to accept this notice electronically. Please email appointment@terryhealthclinic.com to request a paper copy.
You may file a complaint with TerryHealth LLC or with the Secretary of Health and Human Services if you believe your privacy rights have been violated. To file a complaint, send a secure message through your patient portal or email appointment@terryhealthclinic.com. We will not retaliate against you for filing a complaint.
TerryHealth LLC is required by law to maintain the privacy of your protected health information and to provide you with this notice of our legal duties and privacy practices. We reserve the right to change the terms of this notice and will notify you of any changes through your patient portal.
