TerryHealth is a telehealth practice and is not equipped to handle medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately.

2. No Doctor-Patient Relationship Until Established

Visiting this website or completing a contact form does not establish a provider-patient relationship. A provider-patient relationship is only formed once you have completed intake, been accepted as a patient, and had your first visit.

3. Telehealth Scope Limitations

TerryHealth provides care via telehealth only. Some conditions cannot be fully diagnosed or treated remotely and may require in-person evaluation, referral to a specialist, or emergency care. Teresa Vazquez, FNP-BC will determine if your condition is appropriate for telehealth.

4. Scope of Practice

TerryHealth does not provide psychiatric care or controlled substance prescribing for mental health conditions. We do not treat or diagnose psychiatric conditions independently. Patients with mental health needs will be referred to appropriate specialists.

5. No Guarantee of Outcomes

Individual results vary. Testimonials and reviews on this website reflect individual experiences and are not a guarantee of similar results for all patients.

6. Florida Residency Requirement

TerryHealth provides care exclusively to patients physically located in Florida at the time of their visit, in compliance with state telehealth regulations.

7. Not a Substitute for Insurance

TerryHealth operates on a direct-pay model and does not bill insurance. Patients are responsible for understanding that services rendered are not covered by health insurance plans.
